Use this form to request an appointment with us. For new patients, previous eye records MUST be sent to our office for Dr. Lucey to review prior to an appointment being scheduled. Once received, our office will then contact the patient to schedule an appointment.
New patients are seen at 7:45 AM and 1:00 PM. As dilation will be performed, patients are encouraged to bring a driver.
